Good Afternoon.We wish to apply for spaying air for Bonita.Bonita was found several months ago abandoned by owner at Old Kelang Road. She was dehydrated, coughing and slight diarrhea, plus she seems to be off balance, falling down from cage steps and cannot keep focus.Upon visiting the Vet, it is comfirmed she has some kind of vestibular disease and may last a lifetime. We did try to find her a good home but no one wants that kind of burden/responsibility.Other than that and occasional soft poo, she is fine.Now that she has come of age, we send her for spaying.We attach herewith, picture, bill and aid form accordingly.Thank you.Regards,Norvem
